Redesign of Jefferson Discharge Program for Congestive Heart Failure Patients Featured in Philadelphia Inquirer

Hospitals nationwide have been striving to reduce their patient readmission rates. In particular, one in four congestive heart failure patients end up back in the hospital within 30 days of discharge.

Featured in a Philadelphia Inquirer story, Jefferson pharmacists, led by Joanne Heil, have redesigned their discharge program to keep patients from returning to the Hospital. Efforts include calling congestive heart failure patients for 30 days after discharge to ensure that they are taking their medications and staying healthy.
