Jefferson Internal Medicine Associates has achieved the highest recognition (Level 3) for Physician Practice Connections® – Patient-Centered Medical Home™ (PPC-PCMH) from the
National Center for Quality Assurance (NCQA).
The NCQA is a private, non-profit organization dedicated to improving healthcare quality. The Recognition Programs were developed to help clinicians and practices support the delivery of high-quality care.
The Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement. It was developed to assess whether physician practices are functioning as medical homes and recognize them for these efforts.
