We participate in many insurance plans, including Medicare. Please bring proof of your insurance coverage to each visit. Due to insurance company regulations, copays must be made at the time of each visit.

If you are covered by an HMO, remember that, due to HMO regulations, we cannot see you without a referral. If you are uninsured, please discuss payment arrangements with the office manager.
