Pre-Matriculation Health Requirements

New! Attention Students: Jefferson's pre-matriculation health requirements process will change effective 2/8/13. Please carefully follow the guidelines below.

NEW PROCESS: All pre-matriculation health forms and documentation must be submitted through the American Data Bank tracking system. Please register for an account using your jefferson.edu e-mail address. There is a $25 annual subscription fee that can be paid by credit card on the company's website.

All incoming students are required to comply at least 30 days prior to the first day of the semester. Academic programs at Thomas Jefferson University (TJU) are divided into tiers and each tier has different requirements.

Carefully review the documents below to ensure you are on track to complete the process by the deadline.

These documents detail the steps you must follow to complete your forms correctly. All requirements must be completed prior to the first day of the semester in order to be compliant and start your classes on time.

Once the correct tier is determined, if you are in a Tier I or II program, select the Pre-matriculation Physical Evaluation Tier I & Tier II form. For Tier III programs, select the Pre-matriculation Physical Evaluation Tier III form.

If you are an incoming student who is planning to live on the Jefferson campus, you must receive the meningococcal vaccine or complete the Meningococcal Exemption form and submit it via the online process through American Data Bank with your other requirements.

Attention Jefferson Employees!

Jefferson employees matriculating as students are eligible to receive pre-matriculation health services at UHS. Please fill out the first two pages of the appropriate pre-matriculation form and bring it with you to your visit with UHS. Your visit with UHS must occur at least 30 days prior to the first day of the semester to avoid delays.

Episodic Care Sick

Treatment for episodic care is provided to active students of Thomas Jefferson University. Patients are seen on a walk-in basis only. Walk-in hours for sick visits are Monday through Friday, 7:30 a.m. to 11 a.m. and 1 p.m. to 3:30 p.m. If you need to be seen for an affiliate rotation, away rotation or field work physical exam or a travel medicine consultation, please call 215-955-6835 to schedule an appointment.

Annual Tuberculosis Screening and Influenza Vaccination

UHS performs the annual tuberculosis screening and provides the influenza vaccination to all Jefferson students. Students must present their valid Jefferson ID badge to receive these services. Vaccinations

UHS provides the following vaccinations to students for a fee:

  • MMR
  • Varivax
  • Tdap (Adacel)
  • Hepatitis B
  • Menactra

UHS does not take insurance and all services are self-pay. Fees can be paid by cash or check and will be requested by the registration staff prior to having the service performed.

Travel Medicine

Please contact UHS to set up an appointment for a travel medicine consult. Appointments must be made at least 6 weeks prior to the expected date of travel. Travel medicine consults are provided free of charge; however, students are expected to pay for any vaccines, and/or medications prescribed.

UHS has records of students' basic immunizations, i.e., MMR and Hepatitis B. Please bring records of any travel vaccines (Hepatitis A, typhoid) you may have received in the past.

UHS is not licensed to provide the Yellow Fever vaccine. If your itinerary includes travel to a yellow fever zone, we suggest you call 215-955-0860 to make an appointment with Jefferson Travel Medicine Services for your travel consult. You will be charged a fee for this service.

Affiliate Rotation Health Clearance

Students that need a health clearance to be accepted into a clinical rotation at a Jefferson Affiliate should do the following:

  • Call UHS at 215-955-6835 to schedule an appointment for your health clearance visit. Appointments should be scheduled at least 2 weeks prior to your start date.
  • Bring the letter and/or health form provided by the affiliate organization with the UHS visit.
  • If a urine drug screen is required, it must be stated in the letter or the program director should compose a letter to UHS stating this requirement.
  • Be prepared to cover the cost of any health requirements above those required by Jefferson.

Student Personal Counseling Center (SPCC)

SPCC provides counseling for many issues that affect students such as anxiety, depression, relationship problems and academic concerns. All Thomas Jefferson University students are eligible for three free visits to the Center. The Center is located at 833 Chestnut Street, Suite 210. You may call 215-503-2817 to make an appointment.
