Pre-matriculation Health Requirements
All incoming students are required to submit a pre-matriculation health form to UHS at least one year prior to registration. Programs at Jefferson are now divided by Tiers. Each tier has different requirements. Review the Student Cover Letter and Medical Record Requirements by Tier for more information. These documents will detail the steps you must follow to complete your forms correctly.
Once the correct tier is determined, for Pre-Matriculation Physicial Evaluation Tiers I & II Programs, you can download the form here. For Pre-matriculation Physical Evaluation Tiers III Programs, you can download the form here.Jefferson employees may receive this service free of charge. Please fill out the first two pages of the appropriate pre-matriculation form and bring it to our office.
If you are an incoming student who is planning to live on the Jefferson campus, you must receive the meningococcal vaccine or complete the Meningococcal Exemption form and return it to our office with your pre-matriculation health requirements.
