How do I join?  

Participants are asked to complete a detailed questionnaire (available through the links below or through the mail) and may be asked to submit a blood sample and/or cheek swab. The procedure for collection of blood samples and/or cheek swabs is outlined in detail in the Informed Consent form which participants must read and sign before completing the questionnaire.

All participants must sign and return an Informed Consent Form

There are 3 versions of the registry questionnaire:

  1. One to be filled out by the patient

  2. One to be filled out by a family member on behalf of the patient
    (in the event that the patient is deceased or unable complete the form.)

  3. One to be filled out by non-affected family members
    Non-affected family members are first degree relatives (parents, siblings, offspring) or spouses

The questionnaires are designed to elicit the family health history of a patient with pancreas cancer or a non-affected family member, and to document exposure to occupational and environmental factors, such as residential radon, asbestos, and second-hand tobacco smoke.

It is important to sign and return the Informed Consent Form. A signed informed consent form is required from every participant. Each questionnaire takes approximately 30 minutes to complete.
