You have your own personal case manager who will help you and your loved ones handle administrative details (like insurance) and plan for your care after you’ve been discharged from the hospital.

You can reach Case Management at 215-955-2587 or 5-2587 (from inside the hospital), from 8 a.m. to 5 p.m., Monday through Friday.  At other times, you may leave a message.  If you have an urgent requirement during the evening or weekend hours, please speak with your doctor or nurse.
