One way to exercise your rights as a patient is to prepare an advance directive. There are two types of advance directives, a Living Will or a Durable Power of Attorney for Health Care.

In accordance with the Patient Self-Determination Act, you will be asked is you have an advance directive when you are admitted to the hospital. If you have an advance directive, please share it with your doctor and your hospital staff.
