CHR Pediatric Unit, a hospital program specifically designed to help chronically ill children return to the family setting, provides a multidisciplinary approach to the management of medically fragile infants and their families which will improve medical and psychosocial outcomes in a safe environment, at a reduced cost. Located in a separate unit in Thomas Jefferson University Hospital, in close proximity to neonatal and pediatric intensive care units, specialized care is delivered utilizing the full range of pediatric healthcare providers and subspecialty attention. A plan of care is designed for each infant and child to facilitate stabilization and resolution of medical issues, evaluation of educational needs, and an assessment and appropriate alteration of the infant's home environment for homecare. This care plan will also serve to extend the infant's/child's care from hospitalization to their home. This unit is appropriate for infants and children with acute or chronic medical care needs with possible resolution to homecare.
